What we collect
When you request a visit, our form asks for your name, mobile phone number, email, your child's age range, your insurance carrier and any optional notes you add. We also record which page you came from and ad information in the page address (such as campaign tags or a Google click ID) so we know which ads help families find us. Please do not include medical details in the notes box. We'll discuss your child's health with you directly.
How we use it
We use your information only to contact you about scheduling your child's visit and to check your insurance benefits. By submitting the form you agree that we may call or text you about scheduling. Message and data rates may apply. Reply STOP to any text to opt out. We do not sell your information.
Advertising and measurement
This page uses Google Tag Manager, Google Ads conversion tracking and the Meta (Facebook) Pixel to measure how our ads perform. These tools may set cookies or similar technologies and tell Google and Meta that a booking request was sent from an ad visit. Your name, contact details, insurance carrier and notes are never sent to ad platforms. You can limit this through Google Ad Settings, Meta Ad Preferences, or by blocking cookies in your browser.
How we store and share it
Requests are stored securely and are visible only to authorized practice staff. We may pass your details to the scheduling and patient-communication tools we use to book your visit. We keep booking requests only as long as needed for scheduling and our records.
Your choices
You can ask us to see, correct or delete the information you submitted at any time by calling us.
